Wellness Assessment — New Patient Information
Before You Book
Please take a few minutes to read through our appointment details, billing practices, and clinic policies before completing your booking request.
About your visit
The visit
Your Wellness Assessment is up to 90 minutes and is designed for your provider to get to know you, your health history, and your goals. From there, your provider will recommend follow-up lab testing. Full treatment recommendations are typically made after labs are received — this ensures your care plan is based on a complete clinical picture.
Our team
Minneapolis Integrative Medicine Center is a collaborative clinic specializing in integrative and functional medicine. Our providers combine evidence-based clinical care with a whole-body approach to address root causes — not just symptoms. You'll select your preferred provider at booking, but based on your chief concerns we may recommend a different provider to ensure the best possible fit.
Who this is for
This form should only be submitted for yourself, or if you are the parent or guardian of a child aged 13–18. If your child is 18 years old and makes their own medical decisions, please have them submit independently. Do not submit on behalf of a spouse, relative, or friend.
What comes next
Step 1
Wellness Assessment — Your 90-minute new patient visit. You'll review your full health history, symptoms, and goals with your provider, who will outline initial recommendations.
Step 2
Labs — If labs are recommended and you approve them, we will work with you to facilitate specimen collection. No labs are ordered without your explicit permission.
Step 3
Lab review appointment — Once your results are in, we'll reach out to schedule a separate lab review visit to go over findings and next steps with your provider. This appointment has its own fee, separate from the Wellness Assessment and lab costs.
Step 4
Choose your billing model — At the end of your lab review, you'll decide between à la carte billing or a membership plan based on the level of ongoing care that fits your needs. Learn more at mplsimc.com/membership.

Complex Illness
$700
As applicable
What is a complex case?
Complex cases typically include patients with complex chronic illness, a history of cancer, multiple outside records and labs, or those who require coordination with other members of your care team prior to your first visit. These cases require additional clinical prep time for your provider to review your history and get up to speed before you arrive — so they can hit the ground running with you. For that reason, Wellness Assessments for complex cases are billed at $700. We believe this upfront investment expedites your healing journey with us, and we appreciate your understanding.
$200 deposit is charged at the time of booking. This is non-refundable but transfers to a rescheduled appointment. It is forfeited if you do not show up. Deposit funds are separate from any late cancellation fees owed.
Labs
Lab costs are not included in the Wellness Assessment fee. Cost varies based on your symptoms and goals. No labs will be ordered without your express permission.
Lab review
Following your lab results, we'll contact you to schedule a separate lab review appointment. The cost of this appointment is not included in the Wellness Assessment or lab fees.
Insurance
We do not accept insurance, but we do accept HSA/FSA funds. We can provide a superbill for you to submit for potential reimbursement — levels vary by plan and coverage.
Medicare & Medicaid
Patients on Medicare, Medicaid, or any state-assisted insurance programs are not eligible for superbills or insurance-ordered labs. If you're unsure whether this applies to you, please contact us before booking.
Out of state
We are only able to see patients who reside in Minnesota or Wisconsin. Please reach out before booking if you have questions about your eligibility.
Cancellation policy
Wellness Assessment
At least 48 business hours' notice is required to cancel or reschedule. Cancellations within 48 business hours will incur a fee of 50% of the visit cost charged to the card on file.
